The easiest way to access your health information is to sign up for our patient portal, One Chart | Patient. You can access the portal by downloading the Nebraska Medicine app or online.

The fastest way to request your records is through our secure online form. You can: 

• Request copies of your medical records for yourself or someone else.
• Choose electronic or paper delivery.
• Track the progress of your request. 

You will need to provide your full name, date of birth, contact information and details about what records you are requesting. 

If you prefer to request your records by mail, you can complete an authorization form and send it to our Health Information Management team.

You can mail, fax or email the completed form. Processing time varies depending on the type of request. We will contact you if additional information is needed.

Nebraska Medicine participates in secure electronic health information exchanges (HIEs). These exchanges allow your healthcare providers to securely share important medical information with other participating organizations involved in your care.

You may request your health information be amended if you feel the information is not correct. Your request must be in writing and provide rationale for the amendment.
