Request medical records online
The fastest way to request your records is through our secure online form. You can:
- Request copies of your medical records for yourself or someone else.
- Choose electronic or paper delivery.
- Track the progress of your request.
You will need to provide your full name, date of birth, contact information and details about what records you are requesting.
Request medical records by mail
If you prefer to request your records by mail, you can complete an authorization form and send it to our Health Information Management team.
You can mail, fax or email the completed form. Processing time varies depending on the type of request. We will contact you if additional information is needed.
Electronic Health Information Exchange (HIE)
Nebraska Medicine participates in secure electronic health information exchanges (HIEs). These exchanges allow your healthcare providers to securely share important medical information with other participating organizations involved in your care.
HIEs help ensure your care team has access to the most up-to-date information, such as lab results, imaging and medications, even when you see providers outside of Nebraska Medicine.
You have the right to opt out of sharing your information through the HIE at any time. To do so, complete an HIE Opt-Out form, then send it to our Health Information department.
Right to request amendment
You may request your health information be amended if you feel the information is not correct. Your request must be in writing and provide rationale for the amendment.
