For parents

Thank you for your interest in care with us for your child.

We provide family-centered, age-appropriate care to adolescents. Parental or guardian involvement and consent are required for gender care for minors.

To begin gender affirming hormones:

We will need a letter of support from their therapist/counselor to begin gender affirming hormones. Please have them fax the letter to us (fax number below).

We will also need your child to fill out the new patient questionnaire and return it to us as soon as possible.

Once these have been received, your child will be scheduled for a gender assessment visit with our team's psychologist to determine readiness to begin gender affirming hormones. Then they will be scheduled soon after to begin gender affirming hormones.

Puberty blockers only:

If your child is 12 years old or younger, we will only need the new patient questionnaire returned to schedule. If your child is over 13 years old, we will need the new patient questionnaire and a letter of support from your child’s therapist.

Consults only:

This option is available for children younger than puberty ages and is a way to establish a relationship with our clinic and to plan for the years to come.
