Information needed for transfers

To transfer a patient to Nebraska Medical Center or Bellevue Medical Center, please call our Transfer Center and have the following information ready:

  • Patient name and date of birth.
  • Referring facility name.
  • Referring physician's name and contact number.
  • Current level of care.
  • Current vitals and time taken.
  • Current labs.
  • Fax patient demographics to 402.552.3228.

Opening the door to Extraordinary Care

Our Capacity Optimization Hub is a command center for Nebraska Medicine’s  daily hospital operations. It is designed to improve  patient flow and ensure timely, high-quality care.  

This innovative center brings together a multidisciplinary team that works to:

  • Streamline referring facility transfers.
  • Improve patient movement across all levels of care.
  • Facilitate efficient patient discharges.

By placing co-located, interdisciplinary teams in the Capacity Optimization Hub, we can strengthen care coordination, manage patient flow more efficiently, and provide better support for our referring centers—especially when your patients need us most.

Capacity optimization hub: Key teams and function 

  • Transfer center: Nursing staff coordinates requests from referring facilities and manage hospital bed placement for all patients needing admission to Nebraska Medical Center and Bellevue Medical Center. A physician of the day provides medical oversight to all facility transfers.
  • Resource management and staffing coordination: This team oversees centralized scheduling and staffing for the nursing care team.  A nurse resource coordinator adjusts daily staffing levels to meet patient demand.
  • Patient flow management: This group coordinates optimal bed use, manages level-of-care transfers within the hospital, and develops surge action plans to meet incoming patient needs.
  • Logistics and support services: This team optimizes room turnovers to make bed availability faster.  They also manage centralized virtual nursing and patient safety assistance to support bedside nursing care.
  • Discharge SWAT team: This team identifies opportunities to speed up discharges and facilitate transfer-back processes. They also coordinate patient transportation upon discharge.
