Step 1: Understand your coverage
Before beginning your weight-loss journey, it’s important to understand what your insurance plan covers. Not all insurance plans include bariatric surgery or medical weight-loss programs.
You should contact your insurance company and ask about:
- Bariatric surgery coverage.
- Medical weight-loss coverage.
- Any deductibles and co-pays.
- Requirements or criteria for approval.
If they ask for sample procedure codes, you can provide 43644 (laparoscopic gastric bypass) and 43775 (laparoscopic sleeve gastrectomy).
Step 2: Complete your intake forms
Some of our weight-loss programs require you to fill out intake forms before your visit. Please download and complete the form and return it as soon as possible to avoid delays.
