  • If a field is not applicable, write "N/A" or "Not applicable." Do not leave any of the fields blank
• Page two
  • DO NOT sign/date UNTIL you are in presence of a notary
    • If you are under the age of 19, your parent/guardian must sign and date, and have their signature notarized
    • Where to find a notary: Banks, colleges/universities, libraries, FedEx, UPS and law/real estate/tax prep/accountant offices
