Notice to Providers
Helix, Inc. offers a comprehensive population precision health solution, Helix Genomic Advantage™, that enables healthcare organizations, including the participating practices of Health Plans, to improve healthcare outcomes for patients by enhancing utilization of genomics and deploying such knowledge into clinical care. Access to Helix’s genetic test menu is subject to the following terms and conditions:
- Patient Consent. The provision of genetic testing services under the Helix Genomic Advantage Program is subject to Helix obtaining all required informed consents, permissions, and authorizations in accordance with applicable law from each patient to allow (a) the collection of samples, DNA, and genetic material, (b) the performance of genetic testing including whole exome sequencing by Helix, (c) return of genetic testing results to patients, and applicable health care providers, (d) Helix’s storage of a patient’s genetic data for future testing and analysis, and (e) Helix’s use of de-identified remnant samples for ongoing quality assessment and test improvement (“Patient Consent”).
- Specimen Collection. Successful testing services require that participating practices and ordering providers use appropriate specimen collection kits to collect and send specimens to Helix’s laboratory that meet Helix’s criteria for acceptable specimens. Upon request, Helix will provide Helix specimen collection kits and packaging and shipping materials for return shipments to Helix’s laboratory.
- Offsite Use. In-person clinic collection is encouraged as the best practice. However, in the event a participating practice desires to distribute Helix specimen kits to patients for such individuals’ offsite use and individual return of specimens to Helix, or for Helix to deliver Helix specimen kits directly to such individuals (for example, at the home addresses for such individuals), Helix will collaborate in good faith to support such logistics.
- Return of Results. The ordering provider will be responsible for any clinical follow-up, support, and outreach to patients in connection with patients’ results from the testing services.
