Q: What do I need to do to participate in the Chronic Pain Management Program?
A: If your health care provider feels you are a candidate for the chronic pain management program, they will refer you for a pain program evaluation. You will be asked to provide consent to review medical records related to diagnoses, past treatments and mental health history and therapies.
- After your insurance is verified, we will call you to schedule your program evaluation.
- After your evaluation, the program staff will decide as a team if you are a candidate for the program. If you are, your insurance will be verified again for coverage of the program. Once approval is received from your insurance carrier, we will contact you to schedule an admission date.
