You can sign up using the Nebraska Medicine app or the One Chart web portal.  

With an activation code:

Your activation code is provided in your enrollment letter or After Visit Summary (discharge paperwork).

1. Download and open the Nebraska Medicine app or visit onechartpatient.com.
2. On the login screen, tap Sign Up, then Activate your account in the app. Or select Use an activation code in the web portal.
3. Enter your activation code and follow the prompts.

Without an activation code

1. Download and open the Nebraska Medicine app or visit onechartpatient.com.
2. On the login screen, tap Sign Up, then Request an account.
3. Fill out the form and follow the prompts.
