How do I reset my One Chart password?

You can reset your password using the Nebraska Medicine app or the One Chart web portal.

To reset your password:

  1. Download and open the Nebraska Medicine app or visit onechartpatient.com.
  2. On the login screen, tap Get Help, then I forgot my password in the app. Or select Forgot login information? in the web portal.
  3. Enter your information and follow the prompts.

You can also get help accessing your account by calling 402.559.0700.

When you call, our support team may send a secure password reset link to your phone by text message. This message may come from a short code rather than a full phone number. Follow the link in the message to securely reset your password.
