All patients are welcome to apply for financial assistance.
To apply for financial assistance online:
- Log in to the Nebraska Medicine app using your One Chart username and password. You can also log in to the web portal.
- Select Billing from the main menu.
- Select Manage financial assistance from the account overview and follow the prompts.
You’ll need to provide information about your income, expenses and assets. You can upload documents, such as proof of income, as a part of this request. If you don't have all your documents ready, you can save your progress and upload them later.
To apply by mail:
- Download a Financial Assistance Application in your preferred language. You can also email pasfinancialcounselors@nebraskamed.com to request a copy.
- Once you complete the form, mail it to: Nebraska Medicine, 987530 Nebraska Medical Center, Omaha, NE 68198-7530.
Your application must include copies of certain documents that apply to your situation.
