Summary
1. Create a registry of pediatric cancer survivors for the purpose of stimulating future research.
2. Collect socioeconomic demographics and information on access to care to evaluate impact on patient outcomes.
3. Collect patient-reported outcomes on physical activity and overall health to be included in the registry.
4. Collect and store blood to begin a biobank for future research.
5. Consent to allow contact of survivors for future research studies.
