Summary
1. Use electronic health record data to identify and describe adult survivors of childhood cancer who are no longer engaged in survivorship care and compare them with those who remain in survivorship care.
2. Assess differences in adherence to COG long-term follow-up screening guidelines and identify unmet health or psychosocial needs through surveys and chart audits.
3. Explore barriers and facilitators to survivorship care among disengaged survivors through qualitative interviews, with the aim of informing survivor-centered strategies for re-engagement.
