If you forget your username or password, you can reset them in just a few steps.

To recover your username:

1. In the app, tap Get Help , then select I forgot my username.
2. Enter your full name, Social Security number and date of birth.  
3. If your email is associated with your account, your username will be sent directly to your inbox.

To reset your password:

1. In the app tap Get Help, then select I forgot my password.
2. Enter your One Chart Patient | Patient username, Social Security number and date of birth.
3. Select Next and choose to send yourself a security code by text or email.
4. Once you receive the code, enter it and then create a new password.
