Your One Chart | Patient account keeps your health information secure and connects you directly to your care team. If you don’t have a One Chart account, you can sign up in a few ways.
- Check your paperwork. Instructions are included in your After Visit Summary (discharge papers). To set up your account:
- Download the Nebraska Medicine app and select Sign Up on the login screen.
- Sign up online by visiting onechartpatient.com to create your account.
- Call 402.559.0700 and our team can walk you through the setup process.
Here’s what to expect when signing up:
- Share your basic details: You’ll enter information like your name, address, date of birth, phone number and Social Security number.
- Confirm your identity: You’ll answer quick questions to verify it’s really you (e.g., a past address).
- Create your login: Once verified, you’ll set up a username and password to sign in securely.
