Nebraska Medicine Community-Supported Agriculture Box Giveaway
Official Rules
NO PURCHASE NECESSARY TO ENTER OR WIN.
ENTRY PERIOD. The Nebraska Medicine Community-Supported Agriculture Box Giveaway (“Giveaway”) begins on June 10th at 5:00 P.M. CDT and ends on June 10th at 6:45 P.M. CDT (the “Entry Period”).
SPONSOR. The Giveaway is sponsored by The Nebraska Medical Center (“Sponsor”), located at 2120 N 29th Street #200, Omaha, NE, 68111.
ELIGIBILITY. The Giveaway is only open to individuals who are: (i) legal residents of any of the 50 United States or the District of Columbia; (ii) the age of majority in their state of residence, or have the authority to enter into a binding contract under the laws of their state of residence, as of June 10th; and (iii) not family/household members of any employee of Sponsor or its affiliates (each such eligible individual, a “Participant”). This Giveaway is subject to all federal and state laws, rules, and regulations and is void where prohibited. At least four (4) Participants must enter this Giveaway; otherwise, this Giveaway shall be null and void.
HOW TO ENTER. During the Entry Period, there are two (2) ways to enter the Giveaway, as outlined below. Only one (1) Giveaway entry per Participant is allowed.
(a) In-person Entry: A Participant may enter the Giveaway by manually completing an entry slip provided at the Nebraska Medicine booth(s) at Neighbor to Neighbor: A Community Nutrition Gathering ,to be held at 2120 N 29th street #200 Omaha Ne, 68111, and submitting that completed entry slip in-person to a representative of Sponsor at the Nebraska Medicine booth(s) between the hours of 5:00 P.M. and 6:45 P.M. CDT on June 10th 2025.
(b) Alternate Means of Entry (mail in entry): A Participant may enter the Giveaway, without having to complete and personally submit the above-referenced entry slip, via U.S. Mail by hand-printing his or her name, address, city, state, ZIP code, and daytime and evening telephone numbers (including area code) on a 3" x 5" card, and mailing the card to: Attn: Meg Ryan/Community Health to, 2120 N 29th St, Suite 200, Omaha, NE 68111. To enter the Giveaway by this method, you must label the card “Nebraska Medicine Community-Supported Agriculture Box Giveaway” and the card must be postmarked by a date that falls within the Entry Period.
Any entry that is not timely submitted or postmarked, as applicable, is not eligible for entry in the Giveaway.DRAWING. Two (2) prize winners will be selected by a random drawing on June 10th 2025. An official representative of Sponsor will conduct this drawing. Potential winners will be notified in person on June 10th. Potential winners may be required to sign an affidavit of eligibility and release, a publicity release where legal, and Federal and State tax release (or, to cooperate in obtaining such signed documents where necessary), and return these documents, properly executed, via fax, within 48 hours of notification, and the original documents within seven (7) days of date of notification via regular mail. Non-compliance within this time frame and/or with these Official Rules, the inability of Sponsor to contact a potential winner within a reasonable time period, or a prize's or prize notification's return as undeliverable will result in such potential winner’s disqualification and, at Sponsor’s discretion, the applicable prize may be awarded to a potential alternate winner. The prizes described below will be awarded to the declared winner(s) before the conclusion of the event, provided the winner(s) is/are present at that time. If the declared winner(s) is/are not present at the event at the time of the drawing, the prize(s) will be re-drawn and awarded to another attendee.
ODDS. The odds of winning a prize are based on the total number of eligible entries received during the Entry Period.
LIMITATION OF LIABILITY. By entering this Giveaway, Participants waive all right to, and hold Sponsor and its affiliates, and their respective directors, officers, employees, affiliates and agents, harmless from, any claim, liability, loss, damage (including punitive, incidental, and consequential damages), or expense (including attorneys' fees) arising out of or in connection with participation in this Giveaway or the acceptance, use, or misuse of any prize. SOME JURISDICTIONS DO NOT ALLOW THE LIMITATIONS OR EXCLUSION OF LIABILITY FOR INCIDENTAL OR CONSEQUENTIAL DAMAGES, SO THE ABOVE MAY NOT APPLY TO YOU. Sponsor and its affiliates, and their respective directors, officers, employees, affiliates and agents, will not be responsible for: late, incomplete, or incorrect entries; a Participant's failure to receive prize notices due to entrant's spam, junk e-mail, or other security settings or for entrants' provision of incorrect or otherwise non-functioning contact information; technical, hardware, or software malfunctions, lost or unavailable network connections, or failed, incorrect, inaccurate, incomplete, garbled, or delayed electronic communications whether caused by the sender or by any of the equipment or programming associated with or used in this Giveaway; any human error which may occur in the processing of the entries in this Giveaway; or any typographical, technological, or other error in the publishing of the offer, administration of the Giveaway, or announcement of the prizes. If, in Sponsor’s sole opinion, there is such an error, or there is any suspected evidence of tampering with any portion of the Giveaway, or if technical difficulties (including without limitation viruses and bugs) compromise the integrity of the Giveaway, Sponsor reserves the right, in its sole discretion, to cancel or modify this Giveaway in a manner deemed appropriate by Sponsor. In the event of termination, winners will be selected from among all eligible entries received as of date of termination. In the event a dispute arises as to the identity of a potentially winning entrant, entries will be declared made by the name on the entry form.
PRIZES. Two (2) prizes will be awarded (one to each winner). Each prize will consist of a community-supported agriculture box of fruits and vegetables (maximum total retail value of each prize is $30.00). Prizes will consist of only those items specifically listed as part of the prize. No substitution or transfer of prize permitted by winner. Sponsor reserves the right to substitute a prize of equal or greater value. All claimed prizes will be awarded. If a potential winner is disqualified for any reason, Sponsor will award the applicable prize to an alternate winner by random drawing from among all remaining eligible entries. Only three (3) alternate drawings will be held, after which any and all unawarded prizes will remain unawarded. No prize may be substituted for any other form of prize or for cash.
CHOICE OF LAW AND FORUM. Participant agrees that all matters arising out of or relating to this Giveaway and these Official Rules are governed by, and construed in accordance with, the laws of Nebraska, without giving effect to any of its conflict of laws provisions thereof. Entrant further agrees that any legal suit, action, or proceeding arising out of or relating to this Giveaway and these Official Rules shall be brought exclusively in the applicable federal or state courts located in Douglas County, Nebraska.
PRIVACY. All information provided by Participant in an in-person entry or a mail-in entry is available to representatives of Sponsor and may be available to representatives of such other parties as described in Nebraska Medicine’s Internet Privacy Policy, and will be used as follows: to notify Participant of any prize award; to send Participant newsletters from Sponsor and/or their affiliates (where a Participant signs up to receive them); and for such other purposes as described in Nebraska Medicine’s Internet Privacy Policy. WINNERS LIST. For the names of all prize winners, available after June 10th send a self-addressed, stamped #10 envelope to: Attn: Community Health Community Wellness Collaborative, 2120 N 29th St, Suite 200, Omaha, NE 68111.
PUBLICITY. By entering the Giveaway, each winning entrant hereby authorizes (or, as applicable, will cooperate in obtaining an authorization authorizing) Sponsor to announce his or her full name, photo, and likeness and/or statements made by the winner on NebraskaMed.com and/or related websites owned or operated by Sponsor and its affiliates, as well as to publish a post to all social media accounts including Facebook, Instagram, Twitter, Snapchat, any other social media platforms, and any other forms of media.
NO SPONSORSHIP/ENDORSEMENT. This Giveaway is in no way sponsored, endorsed, or administered by or associated with any social networks that are used to share this Giveaway, and Participant completely releases any such social networks from any and all claims related to or arising from this Giveaway. Participant understands that Participant is providing Participant’s information to Sponsor of this Giveaway and not to any social network.
ACCEPTANCE OF RULES. By participating in this Giveaway, Participant acknowledges and agrees that Participant has affirmatively reviewed, accepted, and agreed to all of the Official Rules.
