Release of Patient Information Policy

Nebraska Medicine respects our patients' right to confidentiality regarding the release of information to the public. Patients will be informed of the information that may be released when they sign the Conditions of Admission form. Patients are also told of their right to refuse any release of information.

Condition Reports

Media making inquiries to a patient's condition will be given information according to the family/guardian wishes.

The conditions reports are as follows:

A. Good
Vital signs stable and within normal limits. Patient is conscious and comfortable. Indicators are excellent.

B. Fair
Vital signs stable and within normal limits. Patient is conscious but may be uncomfortable. Indicators are favorable.

C. Serious
Vital signs unstable and not within normal limits. Patient is acutely ill. Indicators are questionable.

D. Critical
Vital signs unstable and not within normal limits. Patient may not be conscious. Indicators are unfavorable.

E. Discharged
Patient has been released from the hospital.

F. Deceased
Identifying information can only be released after next of kin has been notified. (Note: The nursing/medical staff will advise when they feel it is appropriate to release the name of the deceased.) Nebraska Medicine does not release information on organ donation.
