Your Medical Records

Ensuring the confidentiality of your medical records is a high priority for the Release of Information department at The Nebraska Medical Center. The Release of Information staff is available to help make the process of obtaining or sending your medical records as easy as possible. Follow the simple steps below and if you have questions feel free to contact us by calling 402-559-4024 from 8 a.m. to 5 p.m., Monday through Friday.

Completing the Authorization Form

Authorizations are valid for 12 months from the date of signing if no expiration date or identifiable event related to the individual is listed.

The authorization must be completely filled out. Failure to do so may delay the release of your medical information. Incomplete or partially completed requests cannot be processed and will be returned for complete information.

Requests made by anyone other than the patient must include:

  • Proof of legal authority to act on behalf of the patient.
    • Legal proof includes guardianship papers, power of attorney papers, personal representative papers and other legal documents.
  • Signature of the patient’s representative and date
  • Relationship of representative to the patient
Submitting the Authorization Form

You may submit your completed release of information authorization form by mail or by fax. Please remember that incomplete forms cannot be processed and will be returned to the requestor.

Processing Time

Release of Information department requires a minimum of 72 hours or three business days after the written request is received to process requests for medical records. Please allow an additional 7 to 10 days delivery time for records sent by mail. Requests for records created prior to 1999 may take longer to process.

Charges

Federal and state law allows us to charge for medical records. Please indicate on your request form is you would like to be informed of charges prior to copying of the records. The current rates are:

  • Fifty cents per page for the first 50 pages, up to $25
  • Twenty cents per page thereafter
  • Fees are waived if the Release of Information department sends the records directly to a healthcare provider’s office
Sending Your Medical Records to The Nebraska Medical Center

If you plan to see a health care provider at The Nebraska Medical Center and your appointment is scheduled within the next three weeks, you should have your medical records sent directly to the provider’s clinic. Ask a member of the clinic staff to provide you with the mailing address or fax number.

If you are interested in changing primary care providers and want to send previous medical records to The Nebraska Medical Center you must be registered as a patient in our system. This is done by scheduling an appointment with the provider you will be seeing. After you have been registered, have your medical records sent by mail or fax to the provider’s clinic. Patients must be registered in The Nebraska Medical Center system before we can accept records from another provider.
