Patient Support Groups   


The Social Workers are a key part of the transplant team. They address the impact of transplant on aspects of the patient/family lifestyle (such as adjustment to illness/hospitalization, and impact on the family/caregiver, employment, financial issues, etc.) Social Workers may help through:

  • Social Work Evaluation of strengths and needs for patient and family prior to, during and after transplant
  • Supportive Counseling for patient and family, caregiver, children
  • Housing: if your patient has special housing needs or major financial issues with housing and meals.

  • Assistance with Advance Directive information
  • Link with appropriate community resources, including support groups, financial & fund-raising resources, transportation
  • Provide information on support groups
  • Discharge planning assistance, including possible need for medical equipment, home health, rehabilitation, etc.
