You may also be asked to provide a letter of medical necessity to enter our program or for a return visit. If this is required, we offer these templates for download that can be completed by your physician.

Next Steps:

  1. After the referral, a Pre-Transplant Coordinator will contact you to gather information and answer any questions.

  2. An evaluation packet will be sent to your home. The packet contains information about the transplant process and procedure, living donor information, information about our transplant center and questionnaires to be completed and returned.

  3. Medical releases are included in the packet so the transplant center can obtain necessary records from physicians and/or the hospital.

  4. Once we have the questionnaires and medical records, the transplant team will review the information and contact the patient to schedule a time to come to Nebraska Medicine and complete the transplant evaluation.
