A New Life is a support group for all individuals in any stage of the weight-loss surgery journey. Attending this support group serves as the informational session that must be completed before scheduling an appointment for consultation. Attendance is required for all individuals interested in surgical options for weight-loss.

Meeting with this support group gives those considering surgical weight-loss options the opportunity to learn more about the procedures and provides mutual support for patients before and after surgery. We welcome patients at all stages of information about the surgery and all patients who have had weight-loss surgery. Attendees are encouraged bring your family and friends so that they can learn more about the procedures as well.

Information session are held on the 3rd Monday of each month from 6 p.m. to 8 p.m. on The Nebraska Medical Center campus in the Storz Pavilion, Lower level rooms 1-4 which are accessible from Clarkson Tower.

There are two ways to access the meeting site.

• Park in the Clarkson parking lot located on 42nd and Dewey Streets. Enter into the main lobby of Clarkson Tower. If needed, wheelchairs are available at the front desk. Follow the hallway towards the Storz Pavilion Cafeteria. There are elevators that go to the lower level upon entrance into the Storz Pavilion. Go to the lower level.
• On the south side of the parking garage there is a drive leading to the lower level of the Storz Pavilion. The entrance will face east and go directly to the conference room where the meeting will be held.

• Completed Bariatric questionnaire, please print and complete this questionnaire
• A copy of your insurance card – front and back.
• 2 copies of the past two years of your medical history

Bring the completed required documentation with you to the informational session; this will help to get your documentation processed. These documents are required in order for a request to be submitted to your insurance.
