Selecting an insurance plan

Most insurance plans pay only a portion of the cost of your care. You will be responsible for the remaining expense. This remaining expense may include:

  • Deductibles (amount you pay before the insurance plan starts paying)
  • Co-pays (amount payable per visit)
  • Co-insurance (a percentage of the expense that you share with the insurance plan).

Collectively, these are called out-of-pocket expenses. Most insurance plans limit the amount of patient out-of-pocket expenses when services are provided by a provider contracted with the insurance plan. You may have unlimited out-of-pocket expenses if you receive services from an out-of-network provider.

Some insurers are offering 3-tier Preferred Provider Organization (PPO) insurance products. Out-of-pocket expenses are lowest for Tier 1 providers. Out-of-pocket expenses are higher for Tier 2 in-network providers, but still limited to the benefit plan’s maximum out-of-pocket expense. Out-of-pocket expenses are highest, and unlimited, for services provided by non-network (tier 3) providers.

Some insurers offer Point-of-Service (POS) insurance products. Out-of-pocket expenses are lowest for Tier 1 providers, and total out-of-pocket expenses for the patient are limited to the benefit plan’s maximum out-of-pocket expenses. There may be some benefit for out-of-network providers, but the benefits are often very limited, and the patient will have unlimited out-of-pocket expense for out-of-network services provided by non-network providers.

Some insurers are offering Health Management Organization (HMO) or Exclusive Provider Organization (EPO) insurance products. Out-of-pocket expenses are lowest for services provided by in-network providers. In some instances, such as emergency care, there may be some benefit for services provided by out-of-network providers. In most cases, however, there is no insurance plan benefit payable for services provided by out-of-network providers, leaving the patient at risk for very high and unlimited out-of-pocket expenses if services are received out-of-network.

Nebraska Medicine participates with several insurance plans and/or networks. Prior to receiving treatment, please verify with your insurance plan that Nebraska Medicine is in-network for both facility and professional services, prior to receiving treatment. It is also important for patients to verify all physicians providing care are contracted with his/her plan. Hospital services may be contracted however some physicians who provide services at Nebraska Medicine may not be in-network. It is the patient's responsibility to know which providers are in his/her plan.

COMMERCIAL PLANS (NON-MEDICARE / NON-MEDICAID PLANS)

Aetna
Nebraska Medicine is an in-network provider for Aetna insurance plans and for plans utilizing the Aetna Signature Administrators network.
Assurant Health
Assurant Health is the brand name used by Time Insurance Company, John Alden Insurance Company, and Union Security Insurance Company.
Assurant uses several PPO networks. If the insurance plan references Aetna Signature Administrators, Midlands Choice, or First Health, Nebraska Medicine is an in-network provider.

Nebraska Medicine is not an in-network provider for the PHCS or America’s PPO networks. Please check with Assurant Health to verify network status for other PPO networks.
Avera Health Plan Nebraska Medicine is an in-network provider for services authorized by the plan.
Blue Cross Blue Shield Nebraska

Look for a "suitcase" logo on the patient ID card. Nebraska Medicine is an in-network provider when the suitcase logo is on the card.

Nebraska Medicine is not in-network for HMO plans sold by non-Nebraska BCBS affiliates.
Century Health Solutions Nebraska Medicine is an in-network provider.
ChoiceCare / Humana Nebraska Medicine is an in-network provider for PPO plans

Nebraska Medicine is generally not an in-network provider for HMO plans. Contact Humana to verify.
Cigna
Includes Great West, National Association of Letter Carriers, American Postal Workers Union, and other Cigna affiliates.
Nebraska Medicine is an in-network provider for many Cigna health plans. We contract with Cigna via the Midlands Choice network. The Midlands Choice logo may be on the patient ID card. For patients living outside of the Midlands Choice service area, look for Cigna's "Away from Home" logo. Nebraska Medicine is an in-network provider for the Away from Home program.

Nebraska Medicine is not an in-network provider for Cigna's HMO plans.

CoOportunity Health uses the Midlands Choice network for the Premier network; the Midlands Choice logo will be on the front of the ID card.

CoOportunity Health uses the University of Iowa Health Alliance as the primary network for the CorePlus and Preferred products. Although Nebraska Medicine is not part of the UIHA network, CoOportunity Health has added Nebraska Medicine as an in-network provider.

The Midlands Choice PPO network is rented by many insurers and self-insured employers, including CIGNA, CoOportunity Health, and Assurant. Some of the clients that rent Midlands Choice use other PPO networks also. Check with the insurer or claims administrator to verify if they are using the Midlands Choice network.

UMR uses several PPO networks; please check with UMR to identify network providers.

Includes UnitedHealthcare of the Midlands, Golden Rule, UnitedHealthcare of the River Valley, United Medical Security, and numerous other United Healthcare plans.

Optum Behavioral Health, also called United Behavioral Health (UBH), is a United Healthcare subsidiary that manages behavioral health services. Nebraska Medicine professionals (psychiatrists, psychologists) that are in-network providers. Hospital services are not in-network for Optum Behavioral Health.
Wellmark/BCBS Iowa Nebraska Medicine is an in-network provider for PPO plans and other plans that have a "suitcase" logo.

Nebraska Medicine is not an in-network provider for Wellmark’s HMO plans.

MEDICARE PLANS

Nebraska Medicine participates in the federal Medicare program and the railroad Medicare program. We accept all Medicare Supplement plans, including TRICARE for Life. We also take all Medicare Advantage Private Fee-For-Service (PFFS) plans.

Nebraska Medicine participates as an in-network provider for some Medicare Advantage PPO and HMO plans. Some of the common Medicare Advantage plans in the Omaha, Nebraska area are:
