A New Life: The Informational Session
The information session is an opportunity for patients to learn about the bariatric surgery program and the surgical options for weight loss. Attendees will have the opportunity after the session to ask questions. Family and friends are welcome to attend. This information session is provided for all new patients interested in bariatric surgery and must be attended prior to your appointment. There is no fee for this informational session.

Clarkson Tower
Storz Pavilion, rooms 1 and 2
2nd Thursday of each month
6 to 8 p.m.
If needed, wheelchairs are available at the main entrance of Clarkson Tower. Follow the main hallway towards the Storz Pavilion. Inside Storz Pavilion there is a set of elevators to the right. Take the elevator to the ground floor.
Driving to and parking at Clarkson Tower
Use 4350 Dewey Ave., Omaha, NE 68105 in your GPS device.
Please utilize complimentary valet parking Monday through Friday between the hours of 5:30 a.m. to 7 p.m. available in front of the main entrance of Clarkson Tower on 42nd Street and Dewey Avenue. For self-park, please use the orange parking surface lot near the main entrance to the hospital at 42nd Street and Dewey Avenue.

Required Documentation for the Information Session
Complete the bariatric questionnaire
Please print and complete this questionnaire and bring it with you to the session. If you are unable to print the questionnaire, please contact us at 402-559-9500 or by e-mail to have a packet mailed to you.
Bring with you a copy of the front and back of your insurance card.
Please bring one copy of the past five years of your medical records to this session or have your doctor's office fax them to 402-559-3356.
If you see a physician at The Nebraska Medical Center, you will not need to bring them or have them sent to our office.
Special Considerations
Each patient is responsible for contacting their insurance company initially to inquire about bariatric surgery coverage, deductibles, co-pays and policy guidelines or criteria.
Please note that not all insurance programs cover weight loss surgery. It is very important to review your individual policy for bariatric surgery coverage and benefits prior to beginning the bariatric surgery process.
Please do not bring children under the age of 12 to the meeting.
We ask you to arrive 30 minutes early to the meeting to schedule your evaluation and consultation appointment.
If you arrive more than 15 minutes after the session begins, you will be asked to attend another session. It is imperative that you are there for the entire session, as a significant amount of information is discussed.
The Nebraska Medical Center bariatric surgery program office will only obtain insurance pre-certification and pre-authorization for surgery, pre-operative testing and the hospital stay.
