Education is critical to provide diabetes patients with the best possible outcome in managing their disease. Physician referrals can be made to The Nebraska Medical Center Diabetes Center for educational purposes independent of the clinical services available. The Diabetes Center staff will work to create a program for referred patients complementing the treatment program defined by their established primary care physician or internal medicine specialist. Medicare and most third party insurance companies provide benefits for diabetes education.

Please have the patient fill out and sign the medical authorization form.
For diabetes self-management education, please fill out the education referral form.
